    Submit all entries on Blu-ray disc, DVD, MiniDV or USB stick (mpeg2 format). Do not send originals.

    The disc will only be returned if you send SAE when submitting this form. Include your name, your school,  and your
    contact phone number written on the DVD or submitted media.
